Appointments and Communications
An online form submission is a request, not a confirmed appointment. An appointment is confirmed only after direct communication from Jacob AcuBalance. Please use 911 or appropriate emergency services for emergencies.
SMS Messaging Terms

When you separately check the optional SMS consent box on our appointment form, you agree to receive informational text messages from Jacob AcuBalance at the mobile number you provide. Messages may include responses to appointment requests, appointment confirmations, reminders, rescheduling notices, and clinic-related scheduling updates.
