Please do not submit urgent concerns, detailed medical histories, Social Security numbers, payment-card information, or other highly sensitive information through this website.
We may also collect limited technical information such as browser type, device type, approximate location, referring page, pages viewed, campaign parameters, and interaction data through analytics and security technologies.
How We Use Information
We use information to respond to inquiries, coordinate appointments, provide requested clinic communications, operate and secure the website, understand marketing performance, and improve the patient experience. We do not sell or rent personal information.
Mobile Information and SMS Privacy
If you provide your mobile phone number, Jacob AcuBalance may use it only to respond to your inquiry and, when you separately and affirmatively opt in, to send informational appointment communications. These messages may include responses to appointment requests, appointment confirmations, reminders, rescheduling notices, and clinic-related scheduling updates.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. We do not share, sell, rent, transfer, or otherwise provide mobile phone numbers, mobile information, text-messaging originator opt-in data, or messaging consent information to third parties, affiliates, lead generators, or other organizations for marketing or promotional purposes.
All the above categories exclude text-messaging originator opt-in data and consent; this information will not be shared with any third parties. We may use communications service providers solely to transmit messages on our behalf. These service providers may process the information only as necessary to deliver the requested communications and may not use it for their own marketing or promotional purposes.
Participation in the Jacob AcuBalance Appointment Communications program is voluntary. Consent to receive text messages is not a condition of treatment, requesting an appointment, or purchasing any service.
Message frequency varies based on your appointment activity and requests, and is typically no more than four messages per appointment request or scheduled visit. Message and data rates may apply. After you opt out, you may receive one final message confirming that your request has been processed. To receive text messages again, you must provide a new affirmative opt-in.
Appointment Requests and Clinical Information
Website forms, standard email, and standard text messaging may not be fully secure. Information submitted through this website is used for administrative communication and is not automatically part of a clinical record. Clinical records are maintained separately in accordance with applicable requirements.
Analytics, Cookies, and Campaign Data
This website uses Google Analytics 4 and may use similar technologies to understand site usage, traffic sources, and campaign performance.
